To purchase sperm with us, you need to first complete Registration.

If you have reviewed our Donor Catalog and selected a donor, you will need to register to purchase vials. To get started, follow the step-by-step instructions below.

We encourage you to check our donor catalog for donors matching your criteria before registering. While completed registration is required to purchase vials, you do not need to be registered to access our donor catalog online or seek donor details over the phone. Our donor catalog is both diverse and small–recipients will not always be able to purchase vials from their first choice donor. We recommend having backup options available.

To register, print and fill out the forms provided online, and send completed forms back to us via email, mail or fax. For the Informed Consent, handwritten initials next to each statement and a wet signature are required. All other forms can be signed electronically. We only accept PDF documents via email (not JPEG files). Once all forms are received, including the Annual Medical Professional’s Signature form, you will be registered within 3 to 5 business days.

For families with two or more parents, each person trying to conceive is required to register as a TSBC recipient. However, we only charge one registration fee ($100) per family.

Registration steps

  1. Read through our page on choosing your donor for helpful suggestions on that process.
  2. Look at our Donor Catalog and read free profiles and interview notes for each donor. Extended profiles and childhood photos (available for most donors) can be purchased online through the catalog.
  3. Have questions? Call us! We are happy to answer your questions about any of the donors such as:
    • How many pregnancies do they have?
    • How many vials are available for purchase?
    • Can you describe the donor’s adult photo to me?

his step is required even if you plan to ship to your home and/or are planning to inseminate without a medical professional. All recipients are required to have a medical professional on record.

  1. Call us at 510.841.1858 to find out if your healthcare facility is already registered.
  2. If yes, go to STEP THREE.
  3. If your health care facility is not registered with TSBC, you can register them by having them complete our Medical Facility Registration form and submitting the completed form with a copy of their state license OR a voided prescription. There is no fee for medical facility/practice registration.
  4. We can also let you know what medical professionals in your area are already registered with us.

  4. Have your doctor complete the Annual Medical Professional’s Signature Form
  5. If applicable, have your doctor complete the Medical Professional Registration Form and provide a copy of their license/voided prescription.
  6. Mail, fax or email copies of all forms. We can only accept scanned/PDF forms–do not send photographs (ex. jpeg) of forms. Our address, email address and fax number are at the top of EVERY form as well as under Contact/FIND US on the website.  If you send forms by fax, please call us to confirm receipt.
  7. There is a one-time, non-refundable, registration fee of $100 for new recipients (payable by check, Discover, Visa, or MasterCard).
  8. You will receive a Welcome Email from TSBC to let you know your registration is processed and you can now place your first order. It will take 3 to 5 business days to complete the registration process once we receive your complete paperwork.
  9. You may be able to expedite your registration to complete registration within 1 business day (24 hours) from the time we receive all of your forms. The fee to expedite is $100. If you want to expedite your registration, please call us to confirm we have capacity to accommodate you.
  10. If one partner is already registered, and another partner is now trying to conceive, the second partner is required to submit all required registration paperwork and complete registration before using a TSBC Donor. Only one registration fee is charged per family.

Plan ahead

  • If a donor is listed as “Available Soon”, contact us to add you to a donor’s notification list. This will keep you informed about the donor’s next release date.
  • Purchase vials as early as possible to prevent the donor selling out. We will store vials for at least three months at no additional charge. After that, storage fees are charged for vials that remain at TSBC.
  • We encourage you to read our Improving Your Chances of Conception for more information on age and fertility. This resource contains information helpful for all recipients, regardless of age.
  • Schedule shipments and pickup appointments as early as possible. We are a small bank with limited capacity. Scheduling early gives you the best opportunity to ship or pickup on your schedule.
