Client Depositor (CD) for sexually intimate partners or spouse

Storage for use with an intimate partner only

  • Initial Fees: $1700 or $1480 (cancer patients only)

Breakdown of Initial Fees:

  • $350 – Account set up and consultation
  • $400 – Lab processing of sample for storage
  • $400 – STI testing
  • $550 – First year of storage
  • $330 – First year of storage (1 sample only) available for cancer patients only

Additional fees:

  • $400 – Lab/storage appointments to store additional samples (optional)
  • $250 – Wash sample before freezing (optional prep for IUI)
  • $300 to $400 – Shipping fees (standard 2-day, overnight, or Saturday delivery)
  • $25 – Daily tank rental fee for pick-ups (maximum 7 day rental)
  • $75 – Vial retrieval (due for each pick-up or shipment of vials)
  • $550 – Annual storage
  • $330 – Annual storage (1 sample only) for cancer patients only

Directed Donor (DD) quarantine period of 6 months required

6-month quarantine is required. A second exit blood draw (see additional fees below) is required at least 6 months after your last collection appointment..

Initial appointment cost: $2500 

Breakdown of initial fees:

  • $800 – Account set up and consultation
  • $400 – Lab processing of sample for storage
  • $550 – First year of storage
  • $750 – STI testing

Ongoing or additional fees:

  • $400 – Semen Analysis and Test Thaw to assess sperm survival (optional but should be done to confirm the donor’s fertility)
  • $400 – Lab appointments to store additional samples (optional)
  • $250 – Wash vials before freezing (optional prep for IUI)
  • $750 – Exit STI testing (required at least 6 months after last stored sample)
  • $750 – STI (for collections 3 or more months after the initial visit) 
  • $550 and up – Genetic Testing (optional, required for vials being shipped to New York State)
  • $75 – Vial Retrieval (due for each pick-up or shipment of vials)
  • $300-$400 – Tank shipment fees (standard 2-day, overnight, or Saturday delivery)

Known Donor (KD) same as DD except the quarantine period is waived

Because the recommended 6-month quarantine is waived, required to have STI testing (see additional fees below) within 7-days of every sample collected.
