Client Depositor (CD) for sexually intimate partners or spouse

Storage for use with an intimate partner only

  • Initial Fees: $1900 or $1675* (*cancer patients only)

Breakdown of Initial Fees:

  • $400 – Account set up and consultation
    • Donor eligibility determination using FDA, California, and New York state regulations
  • $450 – Lab processing of first sample for storage including semen analysis, freeze/thaw analysis, and other screenings
  • $450 – Regulatory required Sexually Transmitted Infections testing
  • $600 – First year of annual storage
  • $375 – First year of annual storage available for cancer patients who only store one sample/collection.

Additional and ongoing fees:

  • $450 – Lab/storage appointments to store additional samples (optional)
  • $250 – Wash sample before freezing (optional prep for IUI)
  • $350 to $450 – Shipping fees (standard 2-day, overnight, or Saturday delivery)
  • $25 – Daily tank rental fee for pick-ups (maximum 7 day rental)
  • $75 – Vial retrieval (due for each pick-up or shipment of vials)
  • $600 – Annual storage
  • $375 – Annual storage (1 sample only) for cancer patients

Directed Donor (DD) quarantine period of 6 months required

6-month quarantine is required. A second exit blood draw (see additional fees below) is required at least 6 months after your last collection appointment..

Initial appointment cost: $2750 

Breakdown of initial fees:

  • $900 – Account set up and consultation
    • Initial Risk Assessment and document review ($250)
    • Donor eligibility determination using FDA, California and New York State regulations ($550)
  • $450 – Lab processing of first sample for storage, including semen analysis, freeze/thaw analysis, and other screenings
  • $600 – First year of annual storage
  • $800 – Regulatory required Sexually Transmitted Infections testing which includes ABO/Rh factor testing*

Ongoing or additional fees:

  • $450 – Semen Analysis and Test Thaw to assess sperm survival (optional but should be done to confirm the donor’s fertility)
  • $450 – Lab appointments to store additional samples (optional)
  • $250 – Wash vials before freezing (optional prep for IUI)
  • $800 – Exit STI testing (required at least 6 months after last stored sample)*
  • $800 – STI testing (for collections 3 or more months after the initial visit and STI testing) 
  • $650 and up – Genetic Testing (optional but required for vials being shipped to New York State)
  • $75 – Vial Retrieval (due for each pick-up or shipment of vials)
  • $350-$450 – Tank shipment fees (standard 2-day, overnight, or Saturday delivery)
  • $25 – daily tank rental fee for pick-ups (maximum 7 day rental)
  • $600 – Annual storage

*If you pay for exit STI testing upfront, you will pay $750 for each set of testing for a total of $1500

Known Donor (KD) same as DD except the quarantine period is waived

Because the recommended 6-month quarantine is waived, required to have STI testing (see additional fees below) within 7-days of every sample collected.

  • $600 – First year of annual storage

Transfer storage

Transferring vials to store with us from another facility

Initial cost: $1600 

Breakdown of initial fees:

  • $1000 – Account Set Up
  • $600 – Annual Storage
  • If you need us to ship an empty tank to retrieve the vials, shipping is $350-$450

Ongoing or additional fees:

  • $75 Vial Retrieval (due for each pick-up or shipment of vials)
  • $300 to $400 – Tank shipment fees (standard 2-day, overnight, or Saturday delivery)
  • $25 – daily tank rental fee for pick-ups (maximum 7 day rental)
