Donor-conceived adults interested in requesting their donor's identity may use our online contact form. You may also email or call our Executive Director. 510.841.1858 ext204.
If you use the online form, please be sure to include the donor number (if you have it), your parent's names, and your date of birth. Our Executive Director, will explain the process, answer your questions, and send you the necessary forms.
